I give permission for Crown Institute of Higher Education (CIHE) to capture video and audio recordings of online classes conducted via zoom that will include my voice and video. I also consent to publishing the recordings on CIHE’s learning management system (Moodle) that will be accessible to all my peers and lecturer.

I understand that I can revoke this consent at any time in writing and that the use of my voice or videos authorised by this release with immediately cease.
